Community-based health hubs delivering comprehensive primary care services to underserved populations

HHF, along with its charity partner SEED-USA, AMPI-USA, Lean Foundation-USA, Child Aid International Canada, and Wipro Cares, is promoting Universal Health Care as mandated by the WHO. With our 20 Community Health Centres in the most vulnerable urban slums, we are bringing promotional, preventive, curative, and rehabilitative care, along with robust referral pathways, as close as possible without causing any financial hardship to urban poor each year. Located within the neighbourhoods, these centres have grown into trusted spaces where individuals and families can seek affordable care with comfort and continuity.

From the moment a patient walks in, the centres are equipped to address a wide spectrum of health needs. Outpatient consultations through various departments form the foundation, supported by laboratory diagnostics, pharmacy services, and routine health screenings, prompt follow-up that enables early detection and timely care.

Over time, the scope of services has expanded to respond to community needs. Today, CHCs offer multi-speciality care through dedicated General medicine, gynaecology, dental, and ophthalmology services, ensuring that essential specialized consultations are available within the community itself.

Point-of-care diagnostics support, such as X-ray facilities, strengthens clinical assessment, while physiotherapy units provide rehabilitative care for individuals recovering from illness, injury, or mobility challenges, helping them regain independence and quality of life.

Some centres are additionally equipped with inpatient facilities to improve disease management. Day-care units extend this continuum of care by providing facilities for short-term admissions and observation, supporting infusion therapies and monitored treatments. The facility also offers minor procedures through dedicated Minor OT setups, enabling patients to receive timely interventions without prolonged hospitalization.

Anchored in a strong commitment to maternal health, child health, chronic disease screening, and preventive outreach, community engagement, health education sessions, and continuity of care, Community Health Centres play a vital role in strengthening grassroots healthcare access and improving overall community well-being.
