Doorstep NCD care centres bringing sugar, BP screening and chronic care services closer to underserved basti communities.

NCD Sub-Centres, popularly known as Basti Sugar & BP Clinics, were established to deepen community healthcare access beyond existing Community Health Centres. While CHCs serve as comprehensive care hubs, the need was identified to further penetrate underserved slum settlements with smaller, neighbourhood-based facilities focused on chronic disease care.

These sub-centres were designed to bring essential NCD services directly to people’s doorsteps, reducing distance, improving accessibility, and enabling regular monitoring for individuals living with diabetes and hypertension.

The programme begins at the household level through structured door-to-door surveys, helping identify individuals at risk or already diagnosed with non-communicable diseases. This proactive outreach ensures early detection and timely enrolment into care.

At the sub-centres, patients receive routine screening for sugar and blood pressure, NCD consultations, and personalized diet counseling to support long-term disease management. Laboratory investigations are facilitated to enable periodic clinical assessment and treatment monitoring.

When advanced care is required, patients are referred to our Community Health Centres as well as higher referral facilities, ensuring continuity across the care pathway. Dedicated follow-ups are conducted enabling prompt reminders, adherence monitoring, and revisit coordination.

By embedding chronic disease care within basti communities, NCD Sub-Centres strengthen early diagnosis, improve treatment continuity, and preventive & promotive health management among underserved populations.
